For your first appointment, please be sure to bring with you the following:

Your Insurance Information - It is critical to have your insurance information in order to help you in a timely manner.

Your Medical Records - It is important for our physicians to know your medical history. Please bring with you copies of your medical records. It is very helpful for your current physician to send a written summary of your cancer treatment history.

List of Medications - Please bring a list of all over the counter and herbal medications you are currently taking.  Also include dosages and how often you take each medicine.

X-Rays, Scans and Other Films - Please bring the actual films related to your diagnosis. These can be obtained from the imaging department where they were taken. Examples include: X-Rays, CT scans (CAT scans), MRI's, Mammograms, PET scans, Ultrasounds. We have online access to your images if you took them at Main Street Radiology, NYHQ or DOSHI DIAGNOSTICS. Please let us know your name and imaging visit date at your appointment.

Pathology Slides - Pathology slides should be available within 24 hours for your biopsy and/or surgery. Please make arrangement to have the slides with you at your appointment.
